At Little Hearts at Home, we provide compassionate, specialized in-home care for children with complex medical needs through Georgia’s Pediatric Program (GAPP), a Medicaid-funded initiative. As an approved Medicaid provider, we offer both skilled and unskilled private home care tailored to your child’s unique needs.
GAPP is designed to support children under 21 who meet medical necessity requirements, ensuring they receive the care they need without delay. Unlike waiver programs, services can begin immediately once your child is approved, giving families the support they need right when it matters most.
Our intake and evaluation process takes up to 30 days, during which we carefully review your child’s needs to create a customized service plan. Parents play an active role in this process, partnering with our team to develop a plan that aligns with your child’s specific medical and personal needs.

Our Professional Staff are all certified and/or licensed CNA’s LPN’s or RN’s.
Our in-home care services are designed for families seeking professional support for children who need assistance with daily living activities (ADLs) and everyday routines. This program provides caregivers with dependable help throughout the day, making life easier and more manageable for both the child and their family.
From helping your child get out of bed in the morning, assisting with grooming and dressing, to preparing a nutritious breakfast, our team is there every step of the way to start the day off right.
As the day winds down, our professionals can support evening hygiene routines, guide gentle range of motion exercises, and assist with therapy equipment to promote a comfortable, restful night’s sleep.
